2. Size

3. Color or Black and White Ink

4. One or Two-sided

4. Paper Stock

5. If this is an NCR form, how many parts?

      - Does this form need to be numbered?

      - Do these need to be in a book?

      - Do these need a stub and a 

         perforation?
